Clearances

To help chaperone, the following clearances are required:
1. Child Abuse History Clearance (Act 151)
2. The Pennsylvania Criminal Record Check (Act 34)
3. FBI Criminal Background Check (Act 114) (or the affidavit)

PENNSYLVANIA STATE POLICE CRIMINAL RECORD CHECK INSTRUCTIONS

Applicants can also go to the Pennsylvania Access To Criminal History website and apply for their criminal record check online with the Pennsylvania State Police at:

FEDERAL BUREAU OF INVESTIGATION (FBI) CRIMINAL BACKGROUND CHECK INSTRUCTIONS
The Pennsylvania Department of Human Services is utilizing Cogent Systems to process fingerprint-based FBI criminal background checks. The fingerprint based background check is a multiple step process. For more information and to begin the registration process, go to https://www.pa.cogentid.com/index_pdeNew.htm. For question about your FBI Clearance, please contact the FBI/Adam Walsh Unit at 717-783-6211 or 1-877-371-5422.   Please note that you must register prior to going to the fingerprint site.  Walk in service without prior registration will not be provided at any fingerprinting location.

For those who have resided in Pennsylvania for at least 10 years, you will not have to complete the FBI Criminal Background Check if you sign an affidavit.  Click here for the affidavit form.
